Add "test" centre to the list of centres in the questionnaire. For users to experiment with before entering in their info for real

Adding the test centre and re-naming CSIRO and JAMESTEC may have re-numbered the model centres in the questionnaire data base. Need to be aware of this when migrating data to the next beta release.

Gerry, I'm re-assigning this to you because I think we may run into problems when migrating data to the next beta release if the centres are ordered differently now we have new/renamed centres on the list.

....and also on a side note linked to this, it may be better to allow django to force the ordering of example and test centre first (perhaps even slightly separated from the others) as opposed to adding 1 and 2 to the names - would be a tidier solution.

Added test centre and prefixed the example and test centres with numbers so that they appear first on the list r1915
